686 floating hand loose

I recently got a 686 4" Performance Center for a deal I couldn't pass up. I took it out on the range to make sure everything was OK. While shooting I noticed the cylinder doesn't rotate sometimes. After breaking it down and doing some inspecting, I discovered that the floating hand is not catching the extractor star every time. It appears to be loose and not seating properly occasionally. Is this a common issue with these?
