Degree plan reverted back to original and unapproved?

I plan to contact my mentor about this in the morning but I had a meeting with my mentor last Friday and he moved some of my classes around as well as approved my classes for my next term but now it has all reverted back to the original plan and is unapproved again. Has this happened to anyone else? Does my mentor need to make changes and re-approve? Everything was fine just an hour ago, I'm not sure what happened.
