first doctor appointment in ages how do i make them give me hysto 0 waitlist no therapy ?

I'm 19 currently which means that I am finally a legal adult in my state! and I have been for months already I'm just a lazy piece of shit and haven't been to the doctor in over a year and I don't know how to convince them that I need this without being involuntarily hospitalized or forced to wait until I am 35. pls help. I also have been on DIY for almost 10 months now, am I supposed to tell him that or what. He's just a GP but idfk what I'm meant to do. I do not want to switch to prescribed.
