I’m tired of constantly procrastinating, falling behind, and starting over

Advice Needed

I decided to post on this subreddit for the first time because I seriously need some help.

All my life, I’ve generally been on top of my classes, stayed in shape, and been pretty sociable. But med school changed things. The workload got much heavier, and the slacking off and last-minute studying that used to work for me just don’t anymore.

Procrastination is probably one of my biggest problems. I almost never feel enough pressure to actually start something until the deadline is right in front of me. Then I end up doing a bunch of all-nighters, absolutely killing myself for a few days, and somehow still getting average results. I know I’m capable of much more, which makes it even more frustrating.

Every now and then, I’ll go all in on some new “productivity system,” the latest app, a planner, a routine, or whatever I convince myself is going to finally get my life together. It works for a little while, and then I fall back into the same habits.

Honestly, all I want is a balanced life. I don’t need to be #1 in my class, but I’d like to be comfortably in the top 25%. I don’t want to be the biggest bodybuilder around, but I want to be in better shape than the average guy. I want to study, work, exercise, have a social life, and actually enjoy my life without constantly feeling like I’m trying to catch up.

I’m 23, and one of my biggest problems right now is sleep. Scrolling on social media at night feels genuinely addictive to me. I stay up way too late, wake up exhausted, and then I have almost no motivation to do anything. It turns into this cycle of being tired, procrastinating, feeling guilty about procrastinating, and then escaping into my phone again.

I’ve started realizing that I almost feel like I need someone or something to keep me accountable for every decision I make. If nobody is expecting me to do something, I have a hard time making myself do it. I don’t seem to have much internal urgency until the consequences are right in front of me.

Lately, I’ve started feeling like I’m just a casual passerby in my own life. Whatever happens, happens. I rarely feel like I’m actually in control of what I’m doing.

And the weird thing is, I actually love life. I love sports, I love learning, I love being smart and knowing things, and I’ve done things in the past that I was genuinely proud of. I even used to run an online business. Now I sometimes can’t even bring myself to answer customers.

It’s gotten to the point where I almost don’t recognize myself anymore.

Another thing that really bothers me is how tired I look. I have pretty heavy dark circles under my eyes, and I’ve become so self-conscious about them that I sometimes avoid eye contact because I’m worried people are staring at them. I know that sounds superficial compared to everything else, but it genuinely affects how I feel about myself. I’m also worried that I’ve somehow done irreversible damage to myself.

And honestly, I don’t even know exactly what I need at this point. Maybe I need a better routine. Maybe I need to completely change how I approach procrastination. Maybe I need some kind of accountability system or even another person keeping me on track. Maybe I need a mindset shift. Maybe I’m just missing something really obvious.

I know this has probably been asked a thousand times on this subreddit, but I’m really hoping someone who has been in the same shoes can tell me what actually helped them. Not necessarily another perfect productivity system, just practical advice, tools, habits, mindset changes, or anything that genuinely helped you get out of this kind of rabbit hole.

I just want to feel like I’m in control of my own life again.
