I bought the game (type-0) back when it just released and only yesterday got around to opening it up and and entered the code. After entering the code, the demo downloaded automatically with a large update file as well. It should be there still I imagine. If you downloaded it already and deleted it, it should be in your library/download history.

Well, if the rumors are true, there will be Duscae 2.0 soon.
And it used to say "Include FFXV demo on 1st shipping", guess it's time-restricted content.

Episode Duscae 2.0 happened already, and it was an update to the pre-existing demo.
Episode Duscae was indeed a time restricted bonus for digital releases, and lasted 3 months after the release of Type-0 HD. For physical releases, it comes with all first print editions of the game, which you can still find here and there.

Buy it off game fly, I bought it around a month ago and it came with the code.

Probably did. The Demo was meant to be a pre order bonus thing and it lasted for for 2 months on the PS Store. It's well or 2 months so High chance it's removed.
