Someone knew Charlotte had it out for Bethany

Well clearly the person on the tape was indeed Charlotte/Cece. The therapist she's talking to or whatever says "Bethany" not as an interjection to calm the patient down but more as a question to confirm that Charlotte is indeed talking about Bethany.
