Help with fusing values?

so I've been trying to follow the fusing math from the much promoted post last month, but either I'm doing something wrong or the numbers aren't right. For example: I fused a grouping of three Loki 1s (which are common and odd so should be FV2), six punisher 1s (same), and a SW2099 #9 (uncommon 3-6-9 should be FV4). This should be a total of FV22, giving me the chance to get a 2-5-8, 3-6-9, or a premium. Instead I got a Loki 1, which should have been impossible.

Thoughts? Suggestions?
