What determines where you are buried when you die? Do you choose? How is there always enough cemeteries?

I don't understand how in Australia, for example, we can have 6 deaths per 1000 people during the year (and it's way more in other countries I know) yet we don't run out of metropolitan space for the burial of the dead.

What determines where you're buried, and what if there isn't enough room?
