Left testicle feels like a head of cauliflower

22 year old male weighing about 145lbs. It's been like this since at least the beginning of high school. Sometimes I used to get an aching or sore feeling in my testicles when I was younger but it really hasn't happened in a while. There's really no discomfort or swelling, it's just a bit larger than my other one, which looks completely normal and everything seems to work just fine. I do have fairly bad lower back pains, though it could be from playing sports for years. Any idea what's going on?

And no, I haven't had a physical in a very long time. My family doesn't have health insurance so I didn't want them to pay $800 to get myself checked out. They also don't know about this.
