Interesting to see how large exactly is a large saddle PE. Reminds me of the time we coded someone in IR, had a large PE as well from being immobile due to a leg fx. We crashed her there to hopefully dissolve the clot by ditectly injecting tPa on it via cath. I saw from the fluouroscopy that the clot was as big as my wrist watch when my hand was over the patient's chest while we were doing CPR.
