Tips for 100% Lethal, Ghost Run

Most guides online seem to be for Clean Hands runs, which I've already done. I'm looking for tips on getting Ghost while killing everything on highest difficulty. Are there any tips for this kind of run, particularly in taking out multiple enemies grouped together (e.g., wolfhound patrols) before getting Stop Time?
