Ethics and physicians treating family?

I tried a search and did not see anything applicable. Sorry if this was covered somewhere.

What's the consensus on physicians treating family members? Apparently the AMA Ethics Code [discourages] (http://www.ama-assn.org/ama/pub/physician-resources/medical-ethics/code-medical-ethics/opinion819.page) it, but everywhere I have worked it has been verboten.

Had a patient for which a family member (big shot physician) dropped a ton of orders, cancelled a bunch of orders, didn't talk to the attending, and more or less took over the case. No consult, no talking, no nothing. Spent the day repeatedly putting in orders the attending wanted that this physician cancelled. Felt like I was in the middle of a pissing match.

Is this a common thing?

Sidenote: What happens in terms of costs/charges for insurance and Medicare in this situation?
