Surrogate Affidavit Form for the 2016 Democratic Precinct Caucus

So I just realized that I will almost certainly be unable to attend the caucus on the 26th because of work. I found this form on the WA Democratic Caucus website. Is this essentially an absentee caucus voting form, or something else? This whole caucus procedure just seems more and more ridiculous the more I learn about it, especially considering we already have primaries in WA.
