[TOMT] [movie] William H Macy plays a character who goes through illegal means to afford a sex change

Roommate has been trying to figure this out for weeks, doesn't reddit so I'm trying to help. Macy's character attempts to get a sex change for his boyfriend. Going through I believe a loan shark to afford it. There's a scene where Macy's face is pressed to the ground by the boot of the guy he borrowed from. Google searches bring up a few different movies, but none that seem right. He wasn't the central character but the movie was essentially telling a few different stories at once.
