Why is it extremely difficult to plead not guilty by reason of insanity in court cases?

Finally got around to watching Primal Fear. Other movies and tv shows have lead me to believe that it's almost an automatic loss if the defense tries to plead not guilty by reason of insanity. Is the insanity plea even a thing?
