Catacombs of Carthus Co-op bugs?

Right before Wolnir, there seems to be issues with the online play. Often, when trying to summon a phantom I get the "unable to summon phantom" error. Also, sometimes phantoms don't enter the boss area, getting stuck outside the boss room. Are these bugs or am I missing something?
