Adhd, TBI, possible PTSD and employment issues

I'm 28 and suffered my TBI in a car accident at 14. I lost my mom in the wreck, she was only 36 and I've had an overwhelming fear of not achieving my life goals ever since. I fear this more than dying. I'm part of the adhd generation but have really felt it my whole life and have been off meds for 9 years, due to income issues and unwilling doctors. I've always had a deep passion for raising animals and working with pets and livestock. I've struggled with, and not been able to keep, any other work. I've also struggled with low pay and people skills at jobs working with animals. I really don't know where to turn. I've been fighting to get on my feet and help support my kids since splitting up with their mom 2 years ago. I have no job or vehicle and will be on the street in 6-8 weeks.
