Need some help. Can anyone answer some questions for me?

Help A Loved One

This is a throwaway. I don't think my wife would like me discussing this online. So here it goes.

My wife is finishing off her first year at an Ivy League school. When we first moved here she would come home and cry the entire time. She was not on any medication at this point. Eventually after weeks on weeks of her feeling sad and crying almost all day if she was home, crying to the point of not being able to breathe, she went to a doctor here. He slowly started prescribing her more meds. First .5mg ativan as needed and started at 25 or 50mg of lamictal.

It helped and she didn't have major freak outs for as while. So over time she got raised to 200-250mg lamictal along with 1mg ativan and .5 klonopin. Because around finals she would start freaking out and couldn't sleep.

Last night she had a freak out that I hadn't seen in a few months. She came home from studying, and laid in bed. She was hyperventilating, crying and said she was terrified and felt unsafe. She asked me to lay down with her and spoon with her, she says she feels safe with me. But this time she said she wanted to hug me and push me away.

Her anxiety medication did nothing. She took 3 .5mg klonopins over the course of an hour and 1 mg of ativan. But it had no effect.

She told me she was working out, because she was feeling uneasy about studying and needed a break. During her workout she felt the terrified feeling creep in. But tried to exercise through it. That only made it worse.

She was freaking out about her exams this week, she was worried I was being bothered by her neediness, her chest hurt, she was afraid. She was upset she didn't get an A in one class by .4%.

Are these just severe panic attacks? Or is this something else? Should we discuss with her therapist he gets her checked out for something worse?

Sorry if I didn't portray this correctly any questions feel free to ask.

Thank you for your time.

TL;DR Wife pushes herself into a terrified state of panic and crying. Benzos and her mood stabilizers don't seem to help during these moments. Are these panic attacks or something worse that we need to seek help for?

Ninja edit: I also suffer from anxiety and I am also prescribed ativan as needed. I have attacks but they are nothing like this. I know what she's going through for the most part, so I just comfort her and try to talk her down. But last night got me worried.
