After a year my roommate still doesn't do any shared responsibilities. How do I kick him out nicely?

A good friend of mine but a total bum, my roommate never takes out trash or recycling. He's never swept or vacuumed, bought tp or any shared item. I have to be here for any fix the apartment needs and his gf stays here 5/6 nights a week. I've warned him at least 10 times, written him a chore chart and 3 months ago I gave him a "probation period" of 2 months. I work 6 days a week often 12 hours a day and still pull all the weight while he works a beer store his gf hangs out at all day while they read comics 4.5 days a week. How do I tell him he's out by next month, I'm totally exhausted and uncomfortable in my own home. I paid the security dep and signed the lease so he basically rents from me, so how do I say "bye" without ruining our friendship completely?
