Positions On Deathmatch Wrestling?

I never really grew up watching the original ECW so I didn't get the full experience of full on violent, adult wrestling. I was watching a BJW highlight video with John Zandig vs. Tomoaki Honma and the shit they did was insane (I think it was a lemon, glass, salt deathmatch or something or at least those were the items used). But I have to wonder who is crazy or dedicated to their craft enough to do these kinds of things, especially with the dangers of large amounts of blood loss and light tube chemicals. I just can't get into it too much.
