This is potentially a horrible suggestion, but maybe just remove scream?

A grip  
Tightening.

A mouthless  
Calls out  
silent. 

Maybe that sounds dumb though, i fuck around with grammar a bit too much i think, it's starting to affect my speech.

I don't think the first two lines need to be excised - just expand your sensory descriptions. You've got the "touch" down (tight, grip), and the sound (scream, silent). What does the anxiety taste like? look like? smell like? It may seem like an abstract exercise, by you may be able to draw some inspiration from your initial instincts. 

I really felt this poem, but still there is some things i would change.  

I would put "Scream" on the same line as "A mouthless". It just feels like i doesn't fit.  
Also i would change "Silent", to "Silently".

That is it really, other than that i think it is a good strong poem, with a lot of emotion compacted into a few lines. Also i like the title a lot, it already sets the mood for the poem before you start reading.

The use of incomplete, abrupt sentences really captures the feeling of anxiety, and the "grip," followed by the "calling out" and the "silence" accurately represents an anxiety attack, how it can suddenly get you, climax and then be gone, all of a sudden.

Overall, a short but potent poem!

P.S.: IMHO, the first two lines are really important, especially in such a short poem. Do not excise them!
