Does anyone else ever have problems with putting their initials on a prescription?

Every time I count a controlled medication, I double count, circle the number of tablets, and initial the bottle "SM". And it seems like every 2-3 months someone comes in with an empty bottle with my initials and says, "You only gave me 8 pills, but the prescription was for 12." or "I want my other 8 tablets. You wrote 8 on my bottle because you still owe me 8 tablets."

I can't be the only person with an S name that this has happened to...
