Are some of the Titles glitched?

This has probably been mentioned before but anyway so I fought Tadakatsu Honda (Side Mission) without getting hit once and the Title didn't show up? Am I doing something wrong here or do I have to fight a different version of him? Otherwise they need to fix this it's locking me out of +60 reputation.

EDIT 1: I beat him at Main Mission by doing both method, beat him by destroying crystals and actually fighting him both times I didn't get hit and still no Title.

EDIT 2: Decided to continue Main Mission all the way to Okatsu, I beat her without getting hit and I didn't receive the Title as well, WTF!!! is my game glitched?

EDIT 3: I'm convinced that my game is glitched I fought different Bosses, I started at Nue, Yuki-Onna, White Cat, Giant Toad and Ogress all defeated without getting hit and I didn't receive any of their respective Titles, pretty much I lost like 360 points in Reputation which is quite sad and a disappointment meaning I'm locked out of those, I'm hoping that NG+ will fix this glitch this is ridiculous.
