401k Over Contribution

I overcontributed to my 401k in 2016 when I switched jobs mid year. I have since talked to my current employer's plan admin and had them withdraw the excess. Its my understanding that since I withdrew the money before April 15th that I have to report this somehow on the 2016 taxes. When the check comes in the mail does it usually come with a 1099R or is there another way of indicating this withdrawl on my taxes?
