Gamechat broken/Glitch

Two days ago i hop on and all my setting are back to default bur my level is fine. When i went into game chat it doesnt show im talking and showed other were talking but not me. (This is in the lobby and in game) So the next day comes and its a 3 gb update for the game for bugs fixes. So i played normally and thought the chat was fixed. Well now im back at it and its broken again today. I also forgot to mention when I logged on yesterday and updated all my setting were back to how they were. So now i change all setting do everything I restart my whole xbox and still ill go back out and it puts my settings to default and chat is bugged? How do i fix this?!
