Adaptive Helmet when to buy it?

I rarely see Adaptive helmet being used in game, to be honest I don't really understand when it's a good item i think the text is just complicates things and i feel it is the sole reason alot of people also don't purchase it in games. Can someone explain what it actually does? (Couple examples of when and against what would also be good)
