Question about PAL

I passed the exams and got them back. But I have a few questions about the application.

1. On the application it says 'Have you ever changed your name (including by marriage)?'
   My parents did change their last names back in the early 1990s but I believe they did it in Germany. It says for me to write my previous name and (attach a separate sheet if necessary). Does that mean I need to provide a photocopy to prove it?

2. Type of identification. I'm guessing I cannot use a healthcard. It says I can use a citizenship certificate but I only have a citizenship certificate card. They've since been replaced by citizenship certificate papers in 2012. Would the card ID still be valid?

3. Cheques. I only have post-dated cheques ATM. Will they accept a post-dated cheque?

4. Photo ID requirements. The picture that I had taken of me does not show a full front view of my shoulders and I'm wearing a coat. Does that mean the photos that were taken of me are useless?
