[TOMT] [movie] Old movie deadly competition (on an island?)

Like 20 years ago I watched a movie where they put some people on an island or something and they had to fight till one was left and won a price or something. At first they had to sign something and they gave them a surprise pen which stunned them. And on the island they climbed a waterfall or something, and the organizers were watching with hidden cameras.
