Tavistock & Portman "Information for New Patients" document - can someone scan/photograph this and send me a copy? The GIC completely forgot.

I am seriously unimpressed with their admin side -- this comes after they forgot to send me the new patient pack at all, and then discharged me for not returning the enclosed forms that I never received. Two phone calls and a letter later, my referral is underway again and they sent me most of the pack. I have no choice but to persevere.

As far as I can gather, the "Information for New Patients" booklet/leaflet/page/whatever is not confidential or personal and will be the same for everyone. It's something you're meant to hang on to just for informational purposes, so hopefully someone has one lying around. If someone can share their copy with me, then I can get going on this faster than sorting out getting it from the GIC, especially because I have a change of address coming up. You can PM me or post it here or work out something else, whatever you are most comfortable with.
