What do y’all think is the best “commenting system” out there?

It's an interesting design problem: with more than a few comments conversations become convoluted, so how do you design for "best answers" and relevance?

Who does it best right now? Reddit’s method, Facebook’s? Is there a theoretical way to display comments that looks promising but no one has implemented?
