Any techs/owners mind sharing the details, price, and the text of their service contracts with customers?

Haven't done a contract before. Wondering what would be fair in terms of price and other things. Normally I charge $70/hr with a one hour minimum. How much should I charge per computer every month if they have 15-20 computers on site?
