Bloodlance Survival

Toughness-wise my gear is standard I believe but having a hard time already in 65-70's. I know it's a very glassy build but curious if people clearing high GR's are pumping paragon into Vit or what? All I can see on board is that they are using Esoteric and Unity. I don't see anyone swapping out Topaz for toughness gems.
