Anyone have any experience with loan repayment as a benefit from employers namely hospitals?

I'm curious because my coworkers have received offers from hospitals that offer student loan repayment as part of their benefit package. What about me who has paid off all of my student loans but used my 401k and a HELOC to do so? Is it a lump sum or do you have to show an active balance? Thanks for reading!
