Getting the biggest onions from seed?

I'm located northwest of Boston and was wondering what advice New England onion growers have for me in getting the biggest bulbs. Seems that most people grow them from sets but you're limited in the available varieties.

If I were to grow from seed, should I plant them directly in the ground and as early as possible? Or should I start them indoors and transplant them?
