Inquiring why haven't happened yet

Was actually thinking why a nuclear holocaust didn't happen during the cold war. What keeps this madness going, but not mad enough to put an end to itself. Like a parasite that doesn't kill the host but keep it in a perpetual leech mode.
I wonder if there are factors we can't see.
