How far away is far enough for privacy screens?

If unauthorized visitors are in a lobby and can view staff monitors that may access PHI, should they ALWAYS have privacy screens or is there a reasonable distance which the unauthorized persons cannot view the text anymore?

Is a visitor taking a faraway picture of the monitor and zooming in a real threat?
