Disappointed in my friend, who doesn't make any effort to see me. Not sure how I should tell him.

I moved to a new city about two years ago, and about six months in, I met my friend through another friend. We became quickly friends, and had a lot in common.

I have not been seeing him lately, mostly to him not putting in the effort and saying he's always busy. I appreciate our friendship, but frankly I'm disappointed. I'm almost at the point where I want to cut cords. My friendships are important to me, and I get disappointed when they don't put in as much effort as I do. What should I do? I want to tell him I'm not happy with how much effort he has been putting in lately.
