Recently Watched Would You Rather (2012)...[spoilers]

So overall the movie wasn't great but wasn't awful, but the big thing that made me gasp was the ending because of how I misunderstood it. When Iris comes home and goes to wake up her brother, it's revealed he overdosed on pills meaning everything she went through wasn't worth it. That being said when I first saw the face I didn't recognize the brother and gasped out loud because I had assumed that the eccentric billionaire, who had earlier stated, "There's a suitable donor in Romania and arrangements are underway to get your brother the operation," had actually gone and replaced the brother with someone from Romania. That being said I rewatched the scene and it turns out to be much less drastic than I had originally thought. Ideas on the ending I had assumed/would it have been worse?
