Crashing after using Ultimate

I have this problem when I use my ultimate on any champion I play, it crashes after the ultimate ends. https://i.imgur.com/s0q0py3.png If anyone can help me it'd be much appreciated. I have this issue for 2 months now, I play on steam. Edit: This also happens on Smite, only hi-rez.
