Need Help embedding a fillable pdf into site

(If I happen to be asking this question in the wrong subreddit, please let me know, and I'll take my question to a more proper place.)

Hey r/webdev, I'm somewhat of a new dev here, and I need your help with something. I'm developing a new (wordpress) site for an Esports related company (and learning a lot on the go) and I've run into a problem figuring out something I need to do. I have this fillable pdf made in acrobat that twitch streamers are supposed to fill out in order to apply for the stream team.

I basically have 2 options to present this form, of which I know how to do neither: either somehow embed this form onto a page of the site and have people fill it out there, or create a submission form that automatically emails the pdf out to recipients.

I'm aware that both of these 2 options are probably going to require either PHP or JavaScript to do it properly, however I'm not much more than a beginner so far on any coding more complicated than html/css. I'm also not sure if either of these options are possible to do client side or if it needs to be server side.

If you could help explain to me what I need to do, or at least point me in the right direction, I'd be extremely grateful. Thanks again!

Could you not submit the form as standard then take the validated form and generate a PDF of the form based on your existing template using a php library. Many are available to use probably some plugins for wordpress too

I think they'd like to have people fill out the pdf form itself since it's custom made and they want people to see it, unless that's exactly what you're saying and I've misunderstood

Well you would have your standard form that people submit their details too. From the responses you can use a PDF library to generate the PDF how you want it including headers/layout/etc.

I'm just trying to figure out the best way to present this adobe acrobat pdf form to people without having to remake it in any kind of form creator basically

I don't know of a way that is possible. Acrobat is closed source and just editing a PDF requires software to do so. On a webpage your looking at alot more work than is required.

If they've spent a long time on the PDF then your going to spend longer trying to work out a way to display on page, edit, submit and render with correct details.

If you can build a form and submit that then you're already half way there you just need to add your context. If you have PS you can strip the PDF down into header/footer images and loads those in with your form content in the middle with a few lines of code. You probably could've done it in time from original post to now. Look at the library I linked it has all the stuff you need in the examples.
