[Residency] Do I need a letter of recommendation from a medicine chair if I m applying to preliminary programs?

I'm going into radiology and wasn't planning on doing any more medicine rotations, but now I'm wondering if that is a mistake because I don't have a letter with a department head/chair. Help.
