Defended ribbon

When does defended ribbon is granted? Before the current Indianapolis mission never payed attention to it. Now I see that when numerous reds capping and I hit one of them the ribbon is not granted.

Would like to know what are the exact criteria for getting the ribbon

Another question is why yesterday and today more than 50% of my battles are standard battles while as in usual days it's much less frequent?
