Advice On Statement of Claim. Georgia. Chatham County

I am seeking damages against a gas station in magistrate court. I am mostly concerned with the body of text describing the claim. Unless I am found to have significant injury to my skin or eyes, I am most likely going to file this statement of claim (I have two doctors appointments tomorrow; wish me luck!) I have access to a notary, and know how to file this at the courthouse. What are your thoughts on the language used? Any tips you have for me would be greatly appreciated!
