Found a fix for games that show up on your monitor but not headset
I was having difficulty getting the Talos Principle working on my headset; it showed up on my monitor, but my Vive was stuck in a perpetual loading screen. Turns out it was using the wrong graphics card (I play on a laptop, so it was erroneously was using the integrated one for the Vive and 1070 for the game)
-
Right-click on your desktop and open Nvidia Control Panel
-
Find the game that's giving you trouble, and set it so that it uses your primary graphics card.

So alternatively I suppose you could just open Windows 'Display Settings' on Windows 10 by right clicking the desktop, and then choosing your headset at the primary display?
Not in direct mode, which you want to run SteamVR in unless you have a good reason not to (i.e. old TF2 or HL2 VR modes)
