When do effects miss their activation timing?

I play duel links and use the Yubel deck. I don't understand why sometimes, mainly when in a chain or tributed for lava golem/buster blade, yubel terror incarnate effects of summoning ultimate nightmare when leaving the field doesnt work. Is there a consistent condition when the effect and other graveyard effects, such as fire king yaksha, "miss activation timing"?
