Having a weird issue with my belt (I think?), Please help.

Tried adjusting tension off and on, all the electrical wiring seems fine... Not sure what the issue with the belt is. It's certainly noisy, but I thought that was normal until I realized the x axis wasn't working correctly.

Any thoughts on this?
