They say they're diabetic in the imgur comments, so it looks like a case of diabetic neuropathy. Because of restricted blood flow to the extremities, the healing process is greatly slowed and infections can be fatal if not attentively cared for. Also because of the reduced or absent blood flow, portions of the extremities can undergo dry gangrene, where they rot off due to a lack of nutrients.
Yeah... Man, I’m sorry to say it, but they’re almost certainly going to need to amputate that. It’s looking grim for Mister Toe.
Avoiding dealing with it comprehensively isn't going to stop the inevitable.
Get it taken care of and do what your doctors instruct TO THE T.

It looks better in this pic. What caused the sore on the first place?
Did you try to IV between your toes?
No, OP said in earlier posts that she’s diabetic, and this looks like classic diabetic neuropathy.
Edited for falsely assigning a gender when unknown. Thanks, u/Scaramooon!
Correction she* the OP is a she
Thanks, I didn’t know.
Holy Hell! You are a tougher person than I. I would have begged them to remove it by the start of month 2! I do hope you are able to heal feom this & hopefully never have to deal with it again.
My diabetic grandma had a similar problem with her big toe. Her nurse has been using a zinc based cream and it's starting to heal now. Give it a try.
If there is something I have learned from this subreddit is that diabetes is so much more scary and dangerous than I had previously believed.
Seeing posts like these (a lot of other posts much worse than this) and knowing that in 2050 they believe 100 million americans will have diabetes is scary as fuck.
Diabetes is a hereditary thing in my family, but I know that those of us who've spent our lives addicted to soda have dealt with a lot more complications. If you want to avoid it for yourself, I strongly recommend excersizing self control over it!
