Crashing consistently during personal story

Ive had this multiple times during a number of the story events around Orr, multiple times during the attempt to lift off from the cave during victory or death, and currently now on my 5th attempt of trying prisoners of the dragon in HoT right at the very end. It has happened across multiple characters on Borliss Pass.

During dialogue sequences or cutscenes it will either eject me from the game mid-sentence or post-custscene will leave the film reel above my head before ejecting me from the game, requiring the complete restart of the mission.

Ive restarted the client, rebooted my pc, updated my graphics drivers, repaired the client, uninstalled and reinstalled the entire game. Ive had a friend host the instance and I join as a guest and they crash out with me, but can complete them solo with no issues. I'm tired of replaying the same instance over and over, but I want to complete the story.

Anyone know any fixes for this? Is this a known issue? Ive submitted an in-game bug report.
