help needed. TBI and ADHD.

my injury was 22 years ago where I sustained diffuse axonal shearing from a crash on my mountain bike doing 45mph down a hill. survived cause I had a helmet. alive today cause a friend with me went for help. we were in the boonies n everyone had to come by helo or 4x4.

as you can imagine, lots of memory / concentration issues. comprehension difficulties, weirder than normal personality quirks, pain, constant state of exhaustion, depression and problems communicating clearly / effectively that often lead to intense emotions that exacerbate and make all those even worse at that moment. and because of hyper awareness to all my perceived deficits I have become so self-conscious I'm almost a hermit. when in a social situation I cope as I was / still am a natural social butterfly, tho today it's more of a wish, as I am currently hermitting badly.

a little over 3 years ago I was diagnosed with ADHD. I'm on Concerta and it's a wonderful aid. problem is, most TBI and ADHD symptoms are similar and while the medication have helped balance several issues, there are some that appear worse than they used to be (not really, they just stick out more after so many got better). these are causing big issues now...at work, with my relationship with my wife and I'm so overwhelmed that I'm shutting down and hiding from everyone and everything. I dont know how to cope...what's causing what, or if the TBI is exaggregating one thing or if it's the ADHD. at a loss on what strategy or coping mechanism to try for what. I'm a loss for almost everything it seems...

most of this is rhetorical...cathartic in just saying it. sounding it out, getting it into the open, as it were. however, if you know exactly what I'm talking about, feel free to speak up.

I guess a good first step is taking my doctor into confidence, talk about my depression and about whether therapy of some sort would be good.

also, despite all this, I've never been suicidal, so no worries there! :)

lastly, this is not COVID / isolation related either as I've worked from home for a couple of years. it's been stewing in my head for months and I desire to close the distance that's developed between my wife and I.
