The ecms on those are pretty robust,  I don't see issues with them ever.  Definitely check distributor, pull it out check teeth on bottom,  check play in bearings on distributor,  check cap,  rotor,  and module.  If in doubt,  I believe you can get an entirely new one for under 100 bucks

I'll check the Dizzy first. Usually I do an electrical tune up first thing when I get a new truck, but I didn't do it on this one. So could probably use it anyway

Even running a truck straight pipe the 02 sensor should still have what it needs to make your engine run right, at least on these old trucks,  they don't need much information.  If it's an intermittent issue like you are describing,  I'm leaning towards bad sensor,  bad fuel,  or bad distributor

No check engine light ever came on. Could my busted exhaust cause the O2 sensor to go haywire? It came apart right where the manifold meets the y-pipe

Also knock sensor, 02, and coolant temp sensor(2 wire one) will make it run rich as hell and then suddenly not sometimes those could be culprits also.  Knock sensor will typically throw CEL, coolant sensor usually won't.  Heated 02 sensors will throw CEL, single wired ones won't.

Check all your grounds.  I had terrible trouble with my TBI truck when the grounds started going bad.  I ran new ground wires from the battery to frame, battery to engine block, and engine block to frame, then ran a new ground to the intake manifold just for good measure.  All my issues went away after that.

They can certainly wear through - the hoses are a fairly soft material. If along the way one got routed poorly, it could eventually fail and cause a vacuum leak.

Do they clog or leak? That would be indicative of a leak, yeah?

Check all the vacuum hoses? If the engine thinks it's at WOT, it will dump a lot of fuel thinking it needs to be in Power Enrichment mode and generally run like hot garbage. Would also explain all the sucking / hissing noise.

Hey y'all, I have a '94 1500 WT with the 262 and it suddenly started missing on sometimes all cylinders, running rough as shit and could barely go over 25. Pulled over and noticed the injectors absolutely jizzing gas all over the intake (good patterns but way too much) and a really loud hissing or sucking noise from the throttle body area.

Limped like that for maybe 6 miles before it suddenly just smoothed out. 

Gauges showed good voltage and I had plenty of gas. 

Is this most likely a spark issue? Maybe the dizzy jumped a tooth or something? I have a spare pcm from my last truck I can swap, is that a common issue?

Would a bad fuel pressure regulator cause this?

I also had my exhaust rust apart at the y pipe recently but idk if that could cause this sort of behavior.
